My last death would have been to random spikes.

Got caught in an ice storm that was unfortunately placed in a low spot, maybe 1 short step up each way. Thus whenever I tried to escape, I'd slide back down to the middle. On top of that, I couldn't see anything. While I was panicking and sliding around randomly, zombies beat my brains in. Which took a long time because my hired hand was healing me. His brains were beaten in first.
