I have an annoying habit of doing great with exercise for a few weeks, then eventually tapering off and stopping completely. I think I just get bored and thus mentally burned out.
Part of the problem for me is that no matter how good it makes me feel (and it really does), I never find exercise of any kind to be fun. It isn't long before I start to think of it as a "task" and then boom, there goes the motivation.

You probably are doing the same thing over and over again. Avoid isolation exercises like bicep curls, crunches tricep extension. The body doesn't work in isolation so it makes no sense to train the muscles in isolation unless you're working on a muscular imbalance. By doing more multi-joint exercises you can get a much more effective workout in a shorter period of time and don't forget to vary your routine.
For instance, one week focus on the front squats, back squats, overhead squats, take a deload week to let your muscles recover and increase the resistance 10 lbs then start again.
People tend to become less bored if they have measurable progress.

Monday:Shoulder day
Tuesday:Arm day
Wednesday:leg day
Thursday:Abs.
The most effective, functional exercises work multiple regions of the body. So best way to train is to train with full body exercises and then take a rest day. As for cardio, a shorter more intense cardio workout is much more effective for increasing your V02 max(how much oxygen you use) compared to a long, easy cardio workout. Excessive cardio spikes your cortisol levels and cortisol is a catabolic hormone(meaning it breaks down muscle tissue and stores fat). Try doing interval training, 1 minute intense, 2 minutes easy. If you can speak in complete sentences then it is too easy during the intense phase. Never do cardio before resistance training otherwise you'll burn through your muscle glycogen.
Cardio is oxidative meaning it is fueled by fat but your body will still try to use glycogen before resorting to fat and you need that glycogen for resistance training since lifting weights is not oxidative.
Don't focus so much on static stretching either. First warm up(you don't want to stretch cold muscles), then foam roll. Foam rolling is a form of self-myofascial release(SMR) which is a way of applying tension to your muscle fibers as a way to loosen and lengthen them. You always want to do SMR before stretching because if you've got knots in your muscle it doesn't do you any good to pull a knot. After SMR(about 30 seconds per region of the body) do some dynamic stretching. Rather than holding in place for 30 seconds, hold for 3 seconds at a time, relax, and then repeat for 10-15 reps. Focus especially on the hamstrings and pectoral muscles as these will be crucial for the more compound movements and explosive lifts.
