Now that I think about it, in Majora's Mask if you don't stop the moon you get a pretty intense ending.

I'm going to go start playing dark souls right now because of that.

Screw DKC, Rare hit their peak of creepy game overs during the N64 era:

There are some pretty brutal game over screens in Corpse Party on the PSP. Japanese school-kids getting slaughtered in a pretty nasty fashion. 8)

How about the sanity effects in Eternal Darkness? They aren't quite game overs, but they're definitely startling. My favorites are the BSOD and having the protagonist's head spontaneously fall off.

Not all that startling, but as classic as it gets.

Some of the end-game game over screens in MGS 2 were a little jarring though, mostly out of weirdness though.

I agree! Notice how Dick looks at you with his arms crossed before turning into a skeleton and crumbling, as if saying "ok dude, you fucked up really badly this time"

I would go with the infamous game over screen from the bootleg Lion King 5 game, where Simba straight up hangs himself.

Timon and Pumba kill themselves too, but theirs are a little more cartoony.
