There are some pretty brutal game over screens in Corpse Party on the PSP. Japanese school-kids getting slaughtered in a pretty nasty fashion.

How about the sanity effects in Eternal Darkness? They aren't quite game overs, but they're definitely startling. My favorites are the BSOD and having the protagonist's head spontaneously fall off.

haha, you'd think I would have realized that with the Estus Flask being there.
Some of the end-game game over screens in MGS 2 were a little jarring though, mostly out of weirdness though.

I would go with the infamous game over screen from the bootleg Lion King 5 game, where Simba straight up hangs himself.

Timon and Pumba kill themselves too, but theirs are a little more cartoony.
