There's a totally weird and uh... interesting japanese game called "I'm Sorry". I think it's spoofing some notorious politician or something, but yeah. As far as I know, this game hasn't ever been ported to any console. Try that.

Vasara and Vasara 2. Both pretty decent shmups.
I just played Ninja Baseball Batman recently. Ridiculous beat em up.

Strikers 1999 (1945 3) is another that I don't think got ported

Tatsujin II (Truxton II) was only ported to the Marty, but that's expensive and rare enough it almost counts as not being ported at all.

Aqua Jack - I loved this one at my arcade. Unfortunately the stick got messed up and they never fixed it and it sat for months then disappeared. Though this did have a gas pedal, so I guess it sorta doesn't count.
