I'd go with the Duo if you could get one, you probably would have better luck in Japan. I would rather go with the PC Engine/CD-ROM2 if cost is the issue, I just think the way it looks and all hooks together is awesome, even if it has some technical issues.

The PCE/CD-ROM2 combo is not necessarily cheaper (I'm the guy who found a working Duo for $5 once!), but it does look awesome!

Go with the Duo-R, you'll be thanking yourself later. Plus, it's a bit easier to mod for RGB than the others so there's that too.

Why? For those that aren't interested in an RGB mod (like me), what benefit does it have over the Super CD-ROM2 add-on?

It's just that the Duo-R is the most reliable setup of the three. I haven't heard of any problems with the Super CD, but I'd pick a Duo-R just to be on the safe side. The original CD-ROM2 may look nice, but once it dies there's no real way to fix it, aside from getting a new CD drive for it.
