I second getting the genesis 6 pak, Contra Hard Corps, and Ranger X.
Pretty much all the best games you need to have people already said but I'll go a head and say you should pick up Atomic Runner and Road Rash II.

A couple notes for those thinking about a Sega CD:

Earthworm Jim for Sega CD is the best incarnation IMO (better sound).

Eternal Champions for Sega CD is so good and so underrated it's not even funny. One of the best 2D fighters ever in my opinion.

Lots of good games already mentioned. Check out Home Alone. Yes really, that Home Alone, based off the movie. It's cheap and pretty fun. You can always search for gameplay of any of these titles anyone suggested on youtube and see if they are up your alley. Of course you waste can a good evening doing so(like I have), but it's helped me make some purchases and avoid others I knew weren't for me.

This was the first cart I got, its well worth it (Though I got the shit one with Soccer in place of Sonic) in the future you can collect up the versions of the games, but to get you started, greatest cart.
