I wouldn't be surprised to see it in a modern game, since the Japanese influence has gained quite a bit a ground in the U.S. and society is a bit more relaxed about sexual orientation than it was then. The original manual for Mario 2 described Birdo as a boy who thought he was a girl, and preferred to be called Birdetta. At some point the manual was edited to take out the Birdetta line, but the confused boy description remained.
Nowadays, Nintendo (of America) insists Birdo is a girl, but as far as I'm aware Europe and Japan still consider Birdo to be a boy.
Fans argue about it, saying that since the old manual said boy, then boy it is. Other people say that the manual is dated, and what Nintendo says now (girl) is correct.
Like I said, its weird. Its probably easiest to say that Birdo is male to female transgender, which would mean that the correct way to refer to the character is as a girl.
Of course, we're discussing a pink dinosaur who shoots eggs from her mouth.

This is like the Poison case. I was waiting for someone to bring in a Bioware character. I'll just add what I consider to be the best female character in the Mass Effect franchise.
