The poor play testers just never made it that far.
...which is too bad because it is one of the NES's very best games.

Also, if you are looking for classic games with adult-only content, then you should be sure to check out Golgo 13: Top Secret Episode and Golgo 13: The Mafat Conspiracy. Both prominently feature sex(!), smoking(!), and graphic violence.

There is of course the topless harpy in the Kid Icarus manual.

The Metroid manual clearly refers to Samus as male. Apparently Japan didn't send the message and the playtesters never finished the game fast enough.

Hmmm. I thought that was intentional, you know, to further obscure the big reveal.

But how do you explain Justin Bailey? I mean, it was an English password that did you know what. So clearly NOA knew about this aspect, right?

Well, that came out later. The US manual writer probably didn't know that when setting things up to release the game. Even the black Player's Guide says "he". I'm pretty sure it was either the Fun Club or Nintendo Power later that had the JB code. I wonder if it was fixed in the yellow version manual. I've only really dealt with the original silver one.

My understanding regarding Samus's gender is that in the Japanese manual she was referred to using gender-neutral pronouns but English lacks those pronouns, so masculine pronouns were used.

Exactly. Other posters have mentioned some really great examples of games that got past the censorship of the time, but none are Nintendo games and thinking about it, I can't think of any other first party Nintendo game that has something like this.

Wow, sex? Just nudity was a lot for a NES game!

On the Metroid debate, I heard that the translation was intentional, though it may very well be one of those things that you hear so often that you don't even check if it is true.

Couldn't they have just used the name "Samus" instead? Push A and he will jump = Push A and Samus will jump, that kind of thing.

I just can't help thinking this was an intentional obfuscation.

It would be incredibly awkard to repeat "Samus" all the time, I guess.
