If the key board hurts because it's flat, maybe a rounded ergonomic keypad would help. I only know of one brand, nostromo, and I don't know how well it works or how easy it is to setup. Just a suggestion. May not be worth it.
A small table can be used for a keyboard and mouse on the couch. Some fancy mouses actually work on some couches if that sounds comfier.
Is your TV HD? Games will be blurry if not.
What inputs does your TV have? Any VGA, DVI, or HDMI?
