I nominate Viewpoint.

Viewpoint is an excellent SHMUP, with great graphics, and excellent gameplay. The game is from an isometric view, so it may not be technically a shmup, as it has some 3D elements, but it's still an excellent game. The graphics are nice and colorful, and look really sharp for the Genesis. Everytime I play it, it feels fresh and never old, possibly due to the graphics. The graphics are just different enough from an ordinary shmup to seperate it from them. I know, I've talken mostly about the graphics, but the gameplay is great too, but that's not what really sets it apart in my mind.

Outlander and Techno-Cop for the Action/Adventure genre. Violence and gore for violence and gore's sake. I can't help but think of them as a middle finger to Nintendo at that time.

Marvel Land for the platforming genre. Um, I like it.
Yeah, I'll be splitting up a lot and that's probably the most obvious one.
Thanks!

For the Beat 'em Up section: Cyborg Justice

Create a customizable cyborg (when you start a game, you choose a body, legs, and weapon, and there are about 7 of each) and go kill other cyborgs in classic beat-em-up style. Add in the ability to rip enemies apart and steal their weapon or legs (or absorb their body to regain some health) as well as an impressive number of different moves that can be pulled off (impressive for a 3 button controller, that is), and you've got what I consider a true Genesis hidden gem.
I'm known as ZebetiteGlitch or Zeza Von Thantos on other platforms.

I remember seeing that one on the Genesis promotional stuff (boxes/posters) but never actually tried it. I might have to give it a spin...

There was a game show on Nickelodeon I think that featured people playing for high scores and they used to regularly feature Marvel Land.

I nominate Subterrania, fascinating game. It's hard for me to describe as I'm not much of a shooter guy, but HG101 has a nice piece on it:

EDIT:Hey found a video of that old show with Marvel Land

EDIT:Actually, I will say a few words on Subterrania so as not to be lazy. It was rather interesting for its time due to how important physics were to the gameplay. You had to learn how to handle your ship properly so as not to damage it and how to fight gravity on a regular basis by using fuel to boost your ship upwards. These things made the game challenging in a different kind of way than other shooters, where the main challenge comes from combat. Though, the game had that as well. It is one of the purest examples I've seen of a skill oriented game because unlike many other shooters, this game required you to actually get a firm grip on actually HANDLING your ship properly before you could even think about engaging your enemies in combat. Playing the game for the first time felt like the first time you drove a car or rode a bike

Also, you had to rescue people in each stage which meant learning how to fight your enemies without causing casualties. It's these realistic gameplay elements, along with the genuinely skill oriented gameplay that made the game stand out.

Nice soundtrack too, which you don't get to say about too many Genesis games

Subterrania is a game I never hear people talk about, so it's a hidden gem in my eyes

Here's some gameplay:

I love it, though I will admit the controls take a bit of getting used to. Like I said, they crammed a lot of stuff into 3 buttons, and some moves are a lot more effective than others. I'd suggest quickly looking up a move list before playing so you're not just button mashing (which won't get you too far in this game).

I also forgot to mention that it has a two player dueling mode, which is pretty fun, as well as a two player coop (which is pretty standard for beat 'em ups).

Atomic Runner it's sort of run n gun and sort of forced side scroller. Like the way Moon Patrol would let you speed up and slow down but you kept moving.

I am going to nominate "Granada"

This game is a game by WolfTeam put out in 1990, an unconventional shooter that involves you being in an upgradeable tank, freeroaming around the level destroying the bunkers one by one until all are clear and you have to face a giant level boss. This game is amazingly fun, easy to control and not very well known probably because it wasn't very well advertised. Each level is uniquely challenging, and requires quick reactions and pits you against new enemies each level. All of this and a race against the clock makes for a severely under appreciated genesis title.
Got: Atari 2600, Atari 7800Pro, Commodore 64, Odyssey 2, Sega Master System, NES, Genesis Models 1-3, Nomad, Game Gear, Sega CD Model 1, Sega 32x, SuperNES, GameBoys, GameBoy Pocket, GBC, Sega Saturn Model 2, GBA, Nintendo 64, Playstation, Sega Dreamcast, Playstation 2 Slim, Nintendo DS Lite, Xbox 360, Gamecube, PS3 Slim

I loved Cyborg Justice growing up... maybe I tolerated a bit more back then :-) It is very unique and fun, but some parts can be frustrating.

I got lazy in my last post, so I edited it and actually added my personal thoughts on why Subterrania is a hidden gem
