Earthworm Jim 3D. That game was amazingly frustrating, I beat it when I was younger.

I beat Terminator 2 on gameboy (the side-scroller one, not the arcade port). It really wasn't that hard, but I've since seen it on lists of very hard games, so I guess not that many people finished it. It wasn't very good.

The Adventures of Batman and Robin(Genesis)
Shinobi(PS2)

MDK(Dreamcast)
Battletoads in Battlemaniacs
Final Fantasy 12(defeated all optional marks and bosses, some of which took AT LEAST an hour of nonstop playing)
Final Fantasy X(conquered the monster arena)
Ninja Gaiden(Xbox)on Master Ninja difficulty
Ninja Gaiden 2(Mentor mode-got two levels in on Master Ninja mode)
The Dishwasher Samurai(that game was tougher than it looked)

Stranglehold on Hardboiled difficulty

the first time i beat Super Mario Bros. 3 and Super Castlevania 4 was a proud moment for me when i was younger.

I'm always surprised how few people have beaten the original Super Mario Bros. I beat it, and it's a badge of honor. 
Also, I feel awfully good about myself for beating all the challenge stages in Mario Kart DS.

Me too, SMB was one of the first games I owned. So I didn't have much else to play. It was Mario and Double Dragon, both of which I beat

Two words; Ninja Gaiden. Grueling!
I am SO close to beating Super Ghouls n' Ghosts, I can get to the second to the last boss on the second play through, but I ALWAYS run out of time. I've cheated myself and warped to the very last boss, just to make sure I could beat it (I can), but I don't feel good about beating the game like that.
Nethack is impossible. I don't believe it either.
