First, to be courteous, I'd ask Nathan for his permission to use his likeness. I'm sure he wouldn't mind, but it would be a nice gesture.
Second, think about the games you usually see Keith play.
A metal slug type game would be a nicer homage to the character.
You'd also have a lot of options for power-ups. A Rambo power-up, a virtual boy power-up, there are a lot of ways to go about this.
But, whatever you do, you better add Danny McBride as an end boss.

The game shouldn't be like Metal Slug. The goal of the game should be to play Metal Slug. Like, perhaps the laundromat across town just got a Metal Slug arcade cabinet, and Keith, tired of playing on his own Neo Geo cab at home, decided to go across town to try this new one he hasn't played before, but Danny McBride or some other villain doesn't want him to. This sets him up for five themed levels of platforming action!!!
Level 1: Keith's Room
Keith's room is a lot bigger than it looks on the videos. He jumps across gian arcade machines, consoles, his bed, and the floor dodging giant doughnuts, alarm clocks, and corn dogs. The boss is his little sister.
Level 2: The Street
Keith walks down the street, dodging cars, dogs that seem to hate him, birds that seem to hate him, and smiling paper boys that seem to hate him. He also jumps over open manholes and large cracks in the road. The boss is a shriner riding around in a tiny car. Alternatively, it can be a bear dressed as a shriner and riding around in a tiny car.
Level 3. The Shopping Mall
Keith makes his way through a shopping mall. He dodges kids on heelies, security guards who hate him for some reason, kids who throw their ice cream cones at him, and angry old people who try to hit him with their cane. The boss is a dinosaur.
Level 4. The Ice Level
It started snowing while Keith was in the mall. He is attacked by ice crows who hate him for some reason, snow men who hate him for some reason, kids who throw snow balls at him, and random falling icicles. The boss is either a giant snow man or the abominable snow man.
Level 5. The Laundromat
Keith jumps from washing machine to washing machine, trying not to fall in. The ground it hot lava. Deadly soap bubbles rise from the bottom of the screen, and smarter deadly soap bubbles directly try to attack Keith. He also fights bottles of laundry detergent. Some of the washing machines shake and fall into the lava when Keith lands on them, so he has to move fast! The boss is a bubble monster, because Danny McBride would make too much sense.
The game ends with Keith happily playing Metal Slug.

I think the plot should be that Danny McBride stole Keith's Neo Geo and now Keith has to get it back. You could start with a Ghosts n' Goblins homage in the opening. Keith is playing his NeoGeo cab with hearts floating above the two of them, then a satanic Danny McBride swoops down and kidnaps it from him. The adventure begins.
I also recommend Butterfingers and Skitt-els for powerups.

This could actually be quite epic. As Nathan's fanbase grows due to screw attack I'm pretty sure Nathan could make a video requesting the game. Basically describing what he wants (a mix of all the ideas here etc) and then a fan would throw it together in flash.
