For all I know, it could be the Commodore 64 ROM burned onto an NES circuit board. I looked a little online. The japanese version of this is called Aigiina no Yogen: Balubalouk no Densetsu Yori. There are ROMS out there if you want to try it first.

It's a platformer, released for the Famicom in Japan in 1986, but the US version was canceled. The Commodore 64 did see a port of it released in 1988 under the same name, Aigina's Prophecy.
I can think of a few explanations: it's either complete garbage with a fake sticker, a reproduction cartridge, or a prototype created by Vic Tokai, the company that created the game.
I found a website that listed press info on unreleased NES games. Here's what they had for it:
Aigina's Prophecy (Vic Tokai)
* "Coming Soon... Nintendo Entertainment System. May... Aigina's Prophecy (Vic Tokai Inc.)" - Electronic Game Player May/June 1988 (pg 5).
* Play it Again ad - VGCE July 1989 (pg 92).
* $18 / $27: buy/sell price in Funco ad - Game Players Magazine March 1990 (pg 107).

An alternative explanation is that it's the famicom game board plugged into a fami-to-NES adapter shoved inside a cartridge like some of the early NES games.
