Still waiting to to hear names being mentioned when it comes to good FPS games with regenerating health.
America's Army has a nice system. When you get shot non-fatally you start bleeding. If you dont get first aid you'll bleed to your death eventually. First aid only stops you from bleeding but it wont give you any health back.
Single player is quite irrelevant, regenerating health doesn't really hurt the singleplayer experience if it's built that concept in mind. It's multiplayer what you should be concerned about. What you should do is compare CS, Quake, W:ET/TF2 and COD: MW with each other. No way to regain health what so ever/Health and armor pick ups and boosts/Medics/Regenerating health. Given that CoD also magically generates enemies until you rush their spawn point (at least in some places), it's not exactly a realistic game in the first place. If you want realistic, you'd get tired, a small injury would impair performance, and a larger one would mean your involvement in the mission (and the rest of the events depicted in the game) is probably over.

Some of those work well and are found in tough single player games (or single player games with tough sections) and some do not. Some recent games that I think have tough portions are STALKER, Metro 2033, Resistance, FEAR, and Far Cry 2.
That's not how it works in Ys, which was the game mentioned. This argument has nothing to do with difficulty, it's about how it kills the pace of gameplay in multiplayer games. Of course difficulty and regenerating health aren't mutually exclusive, it's just a matter of scaling the game accordingly. Regenerating health in Ys doesn't cost anything Magic-wise.
