It's quite simple. The big red button you see is the pinball's plunger. There's also a button on the left side and one on the right, just like a real flipper. The device -which is a little over 50cm long- connects to your PC with a DIN 5 connection. That's an old keyboard connection, before USB and PS/2. Keep in mind shipping can be quite the dealbreaker with large objects like that. Especially from Europe to the US...
I can tell you I found one on an Italian site for €35 + shipping. That is by far the cheapest one I found...
