Ultima Underworld was way ahead of its time and quite a bit more advanced than Doom on its own (coming out a full year earlier), but I have heard at the time it required a beast of a computer to run whereas Carmack made it so Doom could be run on most PC's fairly easily and was better optimized.
UU set the stage for games like Deus Ex, System Shock 1-2, probably the new Fallout's, and any other first person games you can think of with RPG elements. These often tend to be my favorite kind of games. It's not all about the shooting, there's character interaction and many forms of freedom with how to approach situations.
It's still aged very well, as I just played it earlier this year or last year and got to the end of it. UU2 I hear is even better with more varied environments, I plan to get to it someday, and the spiritual successor Arx Fatalis.
