I ordered Soul Calibur for the DC, which was supposed to be with original case and manual; I got a scratched Soul Calibur disk without any sort of case. Needless to say, game didn't work.
Not a big deal in the end, but I wish I hadn't lost out on 10 bucks.

Bought a supposedly chipped Saturn with a few games plus a ton of burns including a 'genuine' Panzer Dragoon Saga for £80. Thought, bargain! Got it in the post, the system wasn't chipped, Panzer Dragoon Saga was the demo disc with the words demo scrubbed out in black marker. Thought, bastard! So I bought a Saturn for 3 times it's value including the games sold with it. To make things worse, it's one of those 64 pin model 2 Saturns that are an absolute bitch to mod and the burns were all NTSC so they don't work anyway. What a joke.
Luckily I managed to score a HUGE bundle of Saturn games for £14. Sold the dupes, recovered my cost, and gained a lot of good games for the system to boot so it wasn't too bad.

I once bought Mario Party 3 for N64 but received Mario Party 1. The ebay seller was a grandma and I can understand her confusion because there's a 3 inside the dice block on the cover. Fortunately, it wasn't a huge loss.
