My list would be:
1.Games which require a constant online connection even for Single Player

.
2.Paid early access games. Especially those that are going to be "free to play" at release (Children of Tomorrow)
3.Microtransactions
4."Online Pass" codes required to access the multiplayer portion of a game.
5.Unfinished games with massive day 1 patches
6.Games that omit the ability to jump
7.Escort missions with suicidal AI NPCs (Fuck you Ashley

)
8.Invisible Walls (90+% of games ever released

)
9.Cliff hanger endings for unestablished series (Bulletstorm)
10.Poorly implemented third person cameras in a game with tons of tight spaces

.
11.Horrible voice acting choices when dubbing a foreign game. Please just give us text or better a Japanese audio option.
12.AI enemies with unbalanced stats/abilities. Bullet sponges, enemies invulnerable from all but 1 type of damage, and AI who have perfect precision who can one shot you from across the map if you are "detected" even if it's through 5 freaking walls

.
13.Instant fail stealth sections.
14.QTEs pretty much in any game genre. Sometimes they work for "adventure" games like heavy rain but I loathe when I run into them in FPS/TPS.
15. Poorly optimized or none existent aim assist in a console based game where you are intended to use a controller

. (Quantum Theory)
16. Peer to Peer game hosting in competitive multiplayer games.
