Just about every long-running or critically acclaimed show has at least one major stinker in its run. It's just inevitable when you have so many episodes, some of them are gonna be duds. Sometimes, these episodes can be just as well-known as the great episodes.
I'll start with Code of Honor from TNG. Everything, from the racist undertones thanks to the baffling decision to make the actors playing a primitive alien race who lust after strong women black to the terrible fight scene makes me wonder how anybody involved thought this episode was a good idea.

Most of TNG season 1 is pretty mediocre, but this one was particularly embarrassing.
The original script by Katharyn Powers was actually about lizard people with a samurai-esque code of honor. It was director Russ Mayberry that turned it into the racist, sexist mess of an episode that we know today. Mayberry was actually fired during production for different reasons depending on who you you ask, from the creative and casting decisions he made to accusations of being outright racist on the set.
They probably should have scrapped the whole thing but they finished production anyway, probably because they didn't have the time or money to do otherwise.

I watched Shades of Gray. It's a strong contender for "Worst episode in Star Trek".
It gives the most contrived reason for a clip show I've ever seen. "Parasites are reacting to Riker's memories".

It wasn’t great, but it was better than most of the other “cast member has sex with mysterious alien entity” episodes. And it really hurt me to have to type that.

All Star Trek is riddled with daft or crappy episodes which is kind of why I love it. Spock's Brain and Turnabout Intruder are just so shit I can't help enjoying them. I even kind of enjoyed Paris and Janeway becoming salamanders. It's better than being all crappy like Discovery and Picard.
The Simpsons unfortunately has quite a few terrible episodes nowadays, (Lisa Goes Gaga anybody?) but one I really hate is when they did the B-Sharps episode again but this time it was a grunge band (That 90's Show). Hate that one.
Game of Thrones obviously started to rack up an unhealthy good/bad ratio towards the end. The Bells and Beyond the Wall are obvious worst but I didn't like the pointless Season 8 opener, Winterfell. Nothing happens. Even bad episodes had something else in them that made them worthwhile, like the horrible episode Sansa gets raped and her arc reset, that episode still had Olenna verbally abusing Cercei which was always good. In Winterfell they sit around and say maybe something will happen next time. Roll credits.
I also don't like the Futurama with Fry's dead dog because I think it's overly sentimental and I had already heard of Hachi so it felt a bit of a rip-off. Also I have no heart.
EDIT: I forgot the worst one of all: Sherlock's secret evil sister who he beats by playing violin at! Christo Redantor, what a load of bull that final episode was.

I do like this episode, but it never hit me the way it does with seemingly everyone else.
The one that really makes me hurt every time I see it is "Luck of the Fryish". It does a better job of showing the impact of Fry's "death" and it is one of my favorite episodes.
Which is horribly off topic and I'm sorry.
So I'll be generic and say full musical episodes of shows which otherwise aren't musical. They almost always suck, with very few exceptions. I do like the musical episode of That 70s Show.
